Multiple Fully Insured Health/Rx Insurance Options (with included health coaching) Expand

We offer four pre-tax fully insured health insurance options to meet you and your family’s individual needs. No spousal surcharge or force-outs! Team members may add their spouse and dependent children to age 26. Options span from $500 to $2000 single deductible plan offerings. Prescription coverage is included in each plan. Team members working a schedule of at least 17 hours/week are eligible for coverages! These PPO medical plans are accepted by medical providers throughout Michigan, around the country and even overseas! Team members may enroll in medical benefits independently of other offered benefits. The employer is able to provide additional details regarding specific medical & prescription plan offerings.

Health Savings Accounts Expand

If enrolled in a qualifying high deductible health plan, team members are able to contribute pre-tax payroll dollars into a health savings account that is established for their benefit. The established account is owned by the team member and there is no “use it or lose it” associated with the team member’s HSA. Thus, this money can be invested as directed by the team member and has the potential to go in tax free, grow tax tree and be spent tax free – which is a fantastic benefit! The employer is able to provide additional details regarding the HSA offering and associated requirements.

Two Dental Benefit Options Expand

Team members that work a schedule of 17 or more hours/week are eligible for group dental benefits. The team member may enroll in dental coverage for themselves, or the team member may add a spouse and additional dependents to age 26 based on their needs. Each dental plan covers a variety of dental treatments and the Enhanced plan even provides adult and child orthodontia benefits! These dental plan offerings are accepted by dental providers throughout Michigan as well as around the country! Team members may enroll in dental benefits independently of other offered benefits. The employer is able to provide additional details regarding specific dental plan offerings.

Vision Benefits Expand

Team members that work a schedule of 17 or more hours/week are eligible for group vision benefits for themselves, and they may add a spouse as well as dependent children to age 26. This benefit is a nationwide PPO network through the largest provider of vision care in the nation. Team members may enroll in vision benefits independently of other offered benefits. The employer is able to provide additional details regarding specific vision plan offerings.

Accident Insurance Available Expand

Team members that work a schedule of 17 or more hours/week are eligible to enroll in voluntary accident insurance. Team members may enroll themselves or add their spouse as well as dependent children up to age 26. Team members can enroll in this insurance independently from other benefit offerings. This insurance offers an expansive schedule of benefits that pay the subscriber directly in the event of a covered injury, treatment, hospitalization and follow up services. The employer is able to provide additional details regarding specific accident plan offerings.

Critical Illness Insurance Available Expand

Team members that work a schedule of 17 or more hours/week are eligible to enroll in voluntary critical illness insurance. Team members may enroll themselves or add their spouse in this coverage and the team member’s children are automatically covered to age 26. Team members can enroll in this insurance independently from other benefit offerings. This insurance offers an expansive schedule of benefits that pay the subscriber directly in the event of diagnosis of a covered illness. Additionally, every year each covered family member can also receive $50 for getting a covered Be Well Benefit screening test. The employer is able to provide additional details regarding specific critical illness plan offerings.

Company Paid Life Insurance Expand

Team members that work a schedule of 32 or more hours/week are automatically enrolled in Basic Life and Accidental Death & Dismemberment insurance coverage and the monthly premium is paid for by the employer. Coverage amounts will vary based on job classification. Covered team members may add to these coverages voluntarily and may enroll in voluntary spouse and dependent children coverages subject to benefit limitations. The employer is able to provide additional details regarding specific Life and Accidental Death & Dismemberment plan offerings.

Company Paid Short Term Disability Insurance Expand

The employer pays the premiums for team members of certain job classifications for this coverage which replaces a portion of lost income due to qualifying personal illnesses or accidents. The employer is able to provide additional details regarding specific eligibility and coverages.

401K with Generous Company Match Expand

Planning for the future is important. Team members over the age of 21 may choose to make pre-tax and/or after-tax contributions of up to 100% of their wage up to the IRS maximum amount allowable. The employer offers a generous safe harbor match on qualified team member contributions. Participants also have access to the plan’s Advisor for the purpose of investment guidance at no additional cost. The employer is able to provide additional details regarding specific eligibility and plan offerings.

Bonus Pay Opportunities Expand

We offer seniority-based discretionary bonuses as well as other discretionary bonus opportunities to team members in recognition of their service. The employer is able to provide additional details regarding specific opportunities.

Company Paid Licenses and Certifications Expand

Team member costs for a license and/or certification required for their current position will be reimbursed to the team member by the employer so long as the team member is working at least part-time. The employer is able to provide additional details regarding this benefit.

Beyond hands-on in services, team members are encouraged to take advantage of a variety of learning opportunities designed to enhance their knowledge in their given area of interest. The employer offers free CEU opportunities needed by licensed and certified staff as well as many additional resources for those interested in growing in their respective area of interest.

Paid Time Off (PTO) Expand

All full and part time team members earn PTO from their first day worked. New time is accrued based on hours worked and is added to their available balance each pay period. The rate of PTO earned increases based on years of service, and can be used for a variety of reasons. Team members are able to see their PTO balance and usage at all times from their own secure web application. Team members can also make new PTO requests directly from their smart phone or computer using this convenient app. The employer is able to provide additional details regarding their specific PTO offerings.

Paid Company Holidays Expand

Part and full time team members that have completed their introductory period are eligible to receive the benefit of extra paid time for each company holiday provided they meet certain requirements. The employer is able to provide additional details regarding these holiday benefits and associated policy.

Paid Bereavement Expand

The loss of a close loved one may be one of the hardest times that each of us have to face at some point in our lives. At such times we want to ensure that team members have the opportunity of at least temporary relief from their schedules without complete loss of pay. Team members working a schedule of full time are eligible for paid bereavement leave from their scheduled shifts in order to grieve the loss of a family member. The employer is able to provide additional details regarding this benefit and associated policy.

Financial Planning Assistance Expand

Financial planning for the future, including retirement, is an important but sometimes overlooked task for many individuals. This is also one of the most challenging and stressful tasks for many. While individuals may seek guidance from any persons or firms they choose, our 401k enrolled team members can take advantage of access to our plan advisor at no additional charge. Multiple Schedule Options Expand

Due to the 24/7 nature of our environments, there are shifts ranging from day to evening to night. We strive to establish schedule rotations that meet a variety of needs from part time to full time; while still meeting the care and environmental needs of our residents. Shifts are established and modified based on the position type and department needs. The employer is able to provide additional details regarding their specific shift types and availability to accommodate individual requests.
